CHRONIC GASTRITIS:

ETIOLOGY:
In addition to those mentioned in acute gastritis –
Reflux of duodenal contents into the stomach
Associated disease of stomach and duodenum
Chronic hypochromic anaemia.
Immunological factors

Mechanism of mucosal injury is by cytotoxic effects.
Depending on the type of mucosa effected :

Type A gastritis( Autoimmune) – involves body fundus mucosa.
Type B gastritis ( H.Pylori related) – involves antral mucosa
Type AB gastritis (environmental gastritis, chronic atrophic gasritits)
- involves body fundus and antral mucosa
This is the normal appearance of the gastric antrum extending to the pylorus at the right of center. The first portion of the duodenum (duodenal bulb) is at the far right. In the endoscopic views below, the normal appearance of the pylorus is seen at the left, with the first portion of the duodenum at the right.

PATHOLOGICAL CHANGES:
Macroscopically – gastric mucosa may be normal atrophied or oedematous.

Histologically – based on :
extent of inflammatory changes in the mucosa ( superficial or deep)
based on activity of inflammation ( quiescent or active , acute or chronic)
based on the presence of and the type of metaplasia ( intestinal or pseudopyloric

Chronic superficial gastritis
Chronic atrophic gastritis
1) Intestinal metaplasia
2) Pseudopyloric metaplasia
Gastric atrophy
Chronic hypertrophic gastritis.


Uncommon forms of chronic gastritis:
-Gastric candidiasis
-Eosinophilic gastritis
-Haemorrhagic gastritis
-Granulomatous gastritis

CLINICAL FEATURES:
usually causes few symptoms
Nausea , vomiting and upper abdominal discomfort
hypochlorhydria, or achlorhydria and hypergastrenemia in autoimmune gastritis
perinicious anaemia (10%)
