Proving of Abroma Augusta Folia :DISCUSSION

- Muzumdar K P.

The proving was conducted on two groups of provers one coming from Bengal and the other coming from Mysore. Jugal Kishore's proving was conducted on the subjects of Delhi. Considering the disturbances, the climatic condition, the food habits, environmental differences, and temperamental variations, it is very significant to not that very minor alteration three places. All these provings were conducted on double blind method, which exhibits the near identical nature of reactivity of the provers to the drug on trial irrespective of the variations of environment, food habits, temperament etc.

Also these provings were conducted between 1970-73 Ray's fragmentary proving was done in 1919. The time spasm of approximately over fifty years has not materially changed the symptom data.

These two important points establish the validity of the Hahnemannian concept of drug proving as the reliable source of studying the drug activity and its therapeutic application. Can we not infer then that human reactivity to a drug is a constant phenomena.?

From the data presented in the provings it appears that Abroma augusta folia has a special affinity for the mucous membranes and the serous membranes. This is exhibited by the symptoms produced in the head, eyes, nose, the respiratory and the urinary systems. The modalities brought out in the provings reminds one of Bryonia alba. One is prompted to call it an Indian Bryonia.

While trying clinically to apply the drug in a few cases, where B. alba seemed indicated but to which the patient did not respond well, A. augusta folia promptly produced profuse expectoration and relieved the condition suggesting its expectorant properties. This fact is already demonstrated in the provings. However,. this will require further detailed study and clinical experience.

During the course of the proving it was significant to note that in many provers who had shown cysts or ova of helminthic infection, Entamoeba histolytica, Giardia at the commencement of the provings the cysts or ova had disappeared after nine months of provings and the stools did not show the same for a long time thereafter. This indicates that the drug may be possessing anthelminthic properties warranting further detailed investigation.

Headache, sneezing, cough, cold, watering and redness of the eyes, chill, fever, bodyache, malaise and debility indicates its probable usefulness in the early picture of influenza and malaria. It is likely that it may be useful as antiviral and antimalarial in the aforesaid diseases.

Along with the respiratory signs and symptoms such as shortness of breath sneezing, running of the nose etc. the blood picture shows an increase in the eosinophilia count (17) Percent at the cost of lymphocytes (18 Percent) without any appreciable rise in total W.B.C. count (6800/cu mm). This indicates the allergic manifestation and hence it suggests the probable use in allergic conditions such as asthma, allergic rhinitis etc., the disappearance of cysts and ova of helminths also confirm the above observations

In the female provers it showed a capacity to relieve the pain of menstruation and to reduce the tendency to heavy bleeding. In one of the unit nine female provers were employed in this proving. Out of these nine female provers five females were kept as controls. They were in the age group ranging from 20 years to 38 years. Almost all of them had mentioned in their case histories at the commencement of the proving about the heavy bleeding and the attending pain. The eighteen months of proving produced marked improvement in these provers in respect of their bleeding and the pain than in those who were kept as controls.

In another unit ten female provers were employed. Out of these ten female provers, five female provers were kept as controls. They were in age group ranging from 16 years to 32 years. Their menstrual history recorded at the time of commencement of the proving was normal. Eighteen months of proving produced delayed menstruation by ten days and scanty flow-black clotted blood.

None of these provers had history of leucorrhoeal discharge, yellowish white. with offensive, odour, with pain in lower abdomen and back was recorded.

The records of the provings at two different units show the antispasmodic activity of the drug in the uterine musculature. The changes produced in the bleeding pattern of the provers suggest its antihaemorrhagic properties.

Increased thirst, profuse urination, albuminuria, general aching in the extremities, soreness of muscles, debility, itching on the skin with or without eruptions, lethargy and weakness in suggestive of Diabetes mellitus and or Diabetes insipidus. J. K.'s provings have reported actual fall in blood sugar (fasting) level in two of his provers. Ray has accredited this drug to possess antidiabetic potentialities on the basis of his clinical experience. However no such hypoglycemic activity was noticed in the provings. Since the symptoms are suggestive, it is worthwhile to explore the possibility of this drug in that direction.

Soreness in the right hypochondrium with sensation of fullness, loss of appetite, colicky pain, unsatisfactory evacuation of bowels, albuminuria, anasarca, suggests affections of liver and biliary apparatus and its defective functioning.

Desire for sweet has markedly been exhibited. This makes an useful addition to the existing list of drugs. (Argentum nitricum, China, Lycopodium, Sulphur, Calcarea carbonicum).

The varieties of signs and symptoms expressed in the joints and the extremities are suggestive of its usefulness in arthritis and rheumatic pains.

Water soluble portion of the rectified spirit extract of leaves showed no significant pharmacodynamic action upto dose of 50 mg/ml on isolated smooth and skeletal muscles and upto 500 mg/kg in anesthetized dog.

The fact that the proving has brought out symptom complex goes to prove that human experimentation is an ideal media than animal experiments. Human Pharmacology is therefore, the most suited for finer experimentation.

Fraction A of the leaves have shown the acetylcholine like activity. The extract also contains potassium and sodium salts and magnesium in trace. The symptom complex produced have confirmed their activity in the mother tincture.

The trace of magnesium demonstrates its activity on the peripheral circulation and the skeletal muscles. It may be noted with interest that the action of A. augusta resembles that of Magnesia phosphoricum, May be that this proves complementary to each other.

CONCLUSION

Ray (4) found the drug useful in Albuminuria

Amenorrhoea and dysmenorrhoea

Carbuncle

Diabetes mellitus and insipidus

Debility

Enuresis

Sleeplessness

Vertigo

Short proving of Jugal Kishore (6) has indicated its possible uses in

Allergic rhinitis, hay fever and bronchitis

Chilblains

Constipation fissure in ano

Diabetes mellitus and insipidus

Dysmenorrhoea and some other menstrual disorders

Migraine

Rheumatoid arthritis

Sinusitis The present proving while confirming some aspects of the drug's medical action as independently reported by Ray and Jugal Kishore shows that it may also become a useful remedy in such conditions as Headache with sinusitis

Conjunctivitis

Upper respiratory infections Including allergic rhinitis, bronchitis and asthma

Dysentery amoebiasis, diarrhoea, helminthiasis and giardiasis Affections of kidney and liver

Rheumatic pains of joints arthritis, synovitis.

Diabetes mellitus and insipidus Uterine disorders metrorrhagia, dysmenorrhoea.

Further trails will be necessary to define, confirm and stabilise the potentiality of its therapeutic activity.

It mighty eventually prove itself as an important polychrest remedy in the homeopathic armamentarium.
