Vithoulkas has talked about Kali arsenicosum in relation to its fear of heart disease, but he hasn't said much about the rest of the mental symptoms. The first case I will present below is an old case, based in Vithoulkas picture, with little reference to the group analysis, because I did not know what to look for at that time. The second case includes the group analysis, based on the type of questions I had learned to ask by then.

Doing your duty to avoid being made redundant

They feel they always have to do their duty, otherwise things will go very wrong. They never stop, they can't afford to rest, afraid to risk their job or their career. They are equally precise regarding their health, as if they don't take care, things might go very wrong. Their body may dissolve, just as their job may dissolve. They are scrupulous in their observation of the smallest symptoms, in case it is the first sign of something serious. It is their duty to warn the doctor, but if he says there is nothing to worry about, they feel he hasn't taken it seriously and he hasn't done his duty.

Work as a bailiff Their job often revolves around a kind of termination. They may choose to become a doctor, where the the theme of work is around life and death. Or they become lawyers, where they can make judgments around redundancies, inheritances or bankruptcies, or they work as a bailiff where the termination of an era of possession is the central theme.

Rigid and precise

It is their duty to work in a controlled and precise manner. If they make a mistake it may result in someone being wrongly condemned to pay a fine or give up his job. If they are not extremely conscientious as a doctor they might miss the first signs of a fatal disease. They don't tolerate mistakes, either in themselves or in others. And mistakes always happen at the most unexpected moments, usually when their attention has been momentarily diverted.

Expressions Fears: disease, health, felt in the stomach, cancer, heart disease (3!), death, alone (3), others, family, thieves, burglars, poison, murdered, expectations, failure, criticism, opposition, observed, anticipation, constriction; nervous.

Dreams: futile efforts, paralysis, thieves, burglars.

Delusions: dead people, falling, thieves, mistakes, crime.

Irritability: (<) mistakes, (<) contradictions, (<) insult. Mood: complaining, gloomy, (<) exertion, crying. Professions: grocer, civil servant, manager, police officer, clerk, judge, translator, accountant. Causes: loss of job, loss of money, burglary. Generals Type: dark, robust. Locality: right. Weather: cold (3), (>) heat (3), (<) draught. Time: (<) 1 and 3 am (3); periodical, every 2 or 3 weeks. Desires: sweet, sour, vinegar, fruit, lemon, refreshing food, mustard, spices, milk, coffee, warm food, fat; drinks in small sips. Aversion: fat, meat. Food: (<) melon, sour, fruit, strawberry, ice-cream, alcohol, spoilt food. Menses: profuse, frequent or late; (<) menses. Sleep: restless, sleepless, (<) 1 to 4 am. Physical: (<) coition, (<) touch, exertion, (<)(>) motion, (>) sitting bent forwards, (<)(<) tickling (2!). Complaints Weakness, emaciation. Cancer. Leukemia. Sepsis. Stitching pains (3), burning, (>) heat (3).

Discharges copious, watery, acrid, yellow to green.

Oedema, ankles, eyelids, medial upper side (3!).

Headache, left, radiating to eye and ear, (<) stooping, (<) lying down. vertigo. Pain in eyes (<) light. Colds, hay fever; watery, acrid discharge; sneezing, sinusitis. Asthma (<) 3 am, (>) sitting and (<) lying down. Pneumonia with stitching pains in pleura. Affection of heart, pains 93) and fears, palpitations, high B.P., arteriosclerosis, sudden stoppage of heart, low B.P., anaemia. Affections of stomach, pain, ulcers. fear felt in stomach. Nausea and vomiting, little and often. Diabetes. Diarrhoea, little and often, foul odour, with cramps; cholera. Kidney affections, urine profuse. Syphilis. Back ache with sciatica (3), (<) pressure, (<) lying on painful side, (<) menses. Hip problems. Eczema, red, scaling, dry, pustules; psoriasis; thick skin on palms of hands and foot soles. Hair-loss, problems with nails. DD: Ferrum series, Stage 1 and 15. Case A 35 year old man has pains in his chest. He feels there may be something wrong with his heart and it makes him anxious. He has had it checked with a specialist several times, but they can't find anything wrong. From the age of 20 onwards he felt that his heart might give up suddenly. The complaints are worse after he has worked long and hard, but during work he feels better. The pains and fears are worse at 3 in the morning. The pain is paralysing, in the middle of his chest, but not radiating out to any other parts. It usually lasts half an hour. It makes him feel its my turn now. His blood pressure is slightly raised. Other complaints: the skin on his scalp is scaly, with some red patches. He used to stutter as a child. He has always worked hard, busy making sure he would get somewhere in life. When he was a student he allowed himself little time for play, he went through university at the rate of knots and he is now a magistrate in the courts. He likes his work but the pressure is rather high, made worse by the fact that he is very precise and can't tolerate mistakes. He is very restless, can't sit still and do nothing. He is not impatient or easily angered. He is happily married and has three children. He is a member of the church council, which means he has the added burden of guiding his fellow parishioners on matters of faith. He didnt volunteer for this job, but he feels it is his duty to do it well. Generals Type: dark. Weather: slightly coldish, (>) heat and sun.

perspiration: after exertion, on head and neck.

Desires: coffee (3), meat (2), milk (2).

Aversion: cheese (2), endive.

Sleep: good, on abdomen.

Analysis

The fear of heart disease as he describes it is typical of Kali arsenicosum (Vithoulkas). Apart from this important keynote it is interesting to see aspects of both Kali and Arsenicum coming up.

He has always worked hard, and finished his studies in no time.

All he did was work, allowing no time for a break (Kali and Arsenicum). He is restless (Arsenicum), but controlled (Kali).

His job as a magistrate fits this theme beautifully, although I regret that I didnt ask further questions about his motivation and the way he handled his daily duties.

Confirming Kali symptoms: (<) 3 am, B.P. raised, cold. Arsenicum symptoms: precise, restless, fear of death, eczema, red and scaly, dark type, cold, (>) coffee, milk, meat, (<) winter.

Reaction After Kali arsenicosum 1M he had a headache for several days.

Afterwards he feels better, less pain in his chest and much calmer. One month later no more pain in his chest.

Case by Rienk Stuut

A 65 year old woman has had rheumatic pains for the last 8 years, with positive rheumatism test results. She has burning pains in her hands and neck after exertion and often at night. She was the only child of an already elderly mother. She spent 3 years in a Japanese prisoner of war camp with her mother. She was always afraid that her sickly mother would die (Stage 15, Arsenicum). In the camp she always looked after her mother (duty, Ferrum series), afraid that she would lose her (stage 15, Arsenicum).

She accepted the fact that she had to do this as a matter of course (stage 1). Once they were freed the situation got worse because they never knew what the Indonesian people would do with them, and they were threatened all the time. She continually had to control (Ferrum series) her fear (Arsenicum) so as not to attract the attention. When they were finally back in Holland nobody in the family understood what they had gone through and she couldn't talk about it with anyone. She never sleeps well, usually waking up at 4 am.(Kali). It is best when she goes to bed at the same time every night (punctual, Arsenicum). She is slightly obsessive about work. Work (Ferrum series) has to be (Kali) finished, in case I suddenly get ill or die (Stage 15, Arsenicum).

She used to worry that something would happen to the children.

She doesnt like to waste time (Ferrum series). Every week she makes a schedule (Ferrum series) about what to eat on which day and she always buys enough food to last a week, afraid there wont be enough (Arsenicum). She always cooks too much, but never throws anything away.

After Kali arsenicosum the rheumatic complaints gradually disappear and the test results become negative. She sleeps better, she dreams about suppressed feelings of guilt. She decides to do something abut this in her daily life. She has phoned some old friends whom she had lost contact with after a quarrel a long time ago. Later on she got a bout of cystitis, an old symptom she had had before the war.
